What you need:
- A completed DR 2219, Persons with Disabilities Parking Privileges Application completed by a licensed medical professional
- A completed Persons with Disability Placard Renewal Notice, if applicable
- Prepare the required documents as files or photos, then prior to the call please email them to phonetag@larimer.org
