About Us
Health First Colorado, Colorado's Medicaid program, is public health insurance for Coloradans who qualify. There are three types of health plans:
- Physical health plan (which includes vision and prescription drugs)
- Dental health plan
- Behavioral health plan

If you cannot complete the application via the Colorado PEAK website, you may print the Colorado Department of Human Services Application for Public Assistance in English or Spanish.

Completed applications can be dropped off or mailed to our offices listed at the bottom of the page. A face to face interview may be required in order to process your application.
