Dealer Number or Lienholder ID number (if applicable)
State what the agent is authorized to do on behalf of the company
Name of the Authorized Agent(s)
Signature of company representative (must be someone other than the authorized individual)
- A business owner with sole ownership may complete a DR 2444 Statement of Fact form in lieu of providing a letter of authorization
Original, faxed or photocopy is acceptable
