HCP is here to help families with children who have special health needs. We give you helpful information and connect you to services and resources, so it's easier to find the support you need. Special health needs can include physical, emotional, or behavior-related challenges. At HCP, we understand that every family is different. We're here to give you trusted advice and help you coordinate care that fits your family's needs.

Our care coordination services are available at no cost to you—no matter your income or your child’s diagnosis. Our team includes a public health nurse, social worker, dietitian, physical therapist, and a parent consultant. They all work together to support your family.

HCP serves children & youth who:

  • Are age birth to 21 years
  • Have more health and/or developmental needs than other children/youth
  • Live in and/or receive Medical services in Larimer County

    A nonprofit organization in Larimer County that provides short-tern care to children with developmental disabilities.
