Providers: Make a Referral to HCP

HCP helps families of children and youth with special health care needs by providing information and coordinating care. The program connects families to resources and services, making it easier to navigate the complicated systems of support. Special health care needs can include physical, emotional, or behavioral challenges.

HCP care coordination is free for all families, no matter their income or diagnosis. Families can get support from a team that includes a public health nurse, social worker, dietitian, physical therapist, and a parent consultant.

HCP serves children & youth who:

  • Are age birth to 21 years
  • Have more health and/or developmental needs than other children/youth
  • Live in and/or receive Medical services in Larimer County

    Provides free advocacy services for individuals with intellectual and/or developmental disability and their families.
  • Good Health Will
    A nonprofit organization that helps people of all ages gain access to donated medical equipment.
  • Respite Care, Inc 
    A nonprofit organization in Larimer County that provides short-tern care to children with developmental disabilities.
