HCP for Children and Youth with Special Health Care Needs

Providers: Make a Referral to HCP

HCP provides information and care coordination for families of children and youth with special health care needs. The program focuses on referrals, information and care coordination to help families navigate the complicated systems available to support them. A special health care need can be physical, emotional or behavioral. HCP care coordination services are offered at no cost, regardless of income or diagnosis. A public health nurse, social worker, dietitian, physical therapist, and a parent consultant are available to consult with families.

HCP serves children & youth who:

  • Are age birth to 21 years
  • Have more health and/or developmental needs than other children/youth
  • Live in and/or receive Medical services in Larimer County

    A nonprofit organization in Larimer County that provides short-tern care to children with developmental disabilities.
