Infant, Child & Adult Immunizations
Our Immunization Program offers low to no-cost routine and recommended immunizations for infants, children and adults who:
- Have Medicaid
- Have health insurance that does not cover the cost of immunizations
- Do not have health insurance
Our clinical team is available to help navigate what immunizations you are due for and answer any questions. If you do not qualify for low to no cost vaccines, please read the information below about our Private Pay options. Please call 970-498-6700 or complete this form to request an appointment.

Private Pay Vaccine Options
- LCDHE is unable to bill private insurance but we do offer a limited selection of vaccines that require payment in full at the time of service. See list below.
- If you have private medical insurance most vaccines will be covered by your health insurance through your primary care provider (PCP) or local pharmacy that accepts insurance.

Seasonal Flu
Getting a seasonal flu shot is the best way to avoid the worst outcomes of flu, including severe illness, hospitalization, or death. The seasonal flu vaccine, commonly called a “flu shot,” can make you less sick and keep you out of the hospital if you get the flu. Being sick with the flu can make you miss work, so getting a flu shot can help keep you at work to support yourself and your family.
Do you work with poultry and livestock? Getting a seasonal flu shot only protects against seasonal flu and will not protect against avian flu. However, people who are around infected or possibly infected birds or other animals, like farmworkers, should still get a seasonal flu shot. Mpox
Mpox vaccines are available at all of our locations. If you have questions about whether mpox vaccination is right for you, please give us a call at 970-498-6700. Parental/guardian consent is required to administer the Jynneos vaccine to anyone under the age of 18 years. Visit our Mpox webpage for more information.
Immunization Records
You can view and print immunization records for yourself or your child from the Colorado Immunization Information System (CIIS). The CIIS Public Portal can be accessed here. Please review the "How to Use the CIIS Public Portal" document for detailed information on accessing your records: English and Spanish.
Request a Community Clinic
LCDHE is an equity-based community vaccination clinic provider. Our clinic works with local organizations to offer vaccination clinics in our community with the goal of reducing barriers to receiving vaccines. Community-based clinics are currently serving the following populations:
- Uninsured or underinsured individuals ages 6+
- Individuals with Medicaid ages 6+
- Individuals requesting COVID-19 Vaccination
