Step 2  

What do I need?

  1. If the applicant’s passport has been lost or stolen, and the applicant is age 15 or under, both parents and the applicant must be present at the time of the appointment

     

  2. If the applicant’s passport has been lost or stolen, and the applicant is age 16 or 17, one parent and the applicant must be present at the time of the appointment

     

  3. If the applicant’s passport has been lost or stolen, and the applicant is age 16 or 17, one parent and the applicant must be present at the time of the appointment

     

  4. DS-64 Statement Regarding a Valid Lost or Stolen U.S. Passport:
    • If your current valid passport was lost or stolen, please visit the US Department of State website and complete the section for Report Lost or Stolen. Print the DS-64 form after completion and bring it with you to your appointment.
    • Completed application (Form DS-11):
      • Complete your application on the US Department of State website and print it single-sided
        • Paper applications are available in our office. Please arrive at least 15 minutes before your appointment to complete them
  5. Evidence of Citizenship: 
    • Original or certified copy of birth certificate, certificate of naturalization, or consular report of birth abroad
  6. Identification: Physical government-issued photo identification for both the applicant and the parent. Acceptable forms of ID include:
    • Valid (non-expired) driver’s license or state-issued ID card
    • US military ID
    • US or foreign passport
    • US permanent resident card
    • If your ID is a non-driver state-issued ID card or instruction permit, or if your driver’s license is from a different state, please provide a second form of ID such as a student ID, Costco card, gym membership, etc. (preferably a photo ID).
    • A complete list of acceptable forms of identification can be found at the US Department of State website.
  7. A passport photo can be taken during your appointment for $15. You can also have the photo taken elsewhere (Staples, FedEx, Walgreens, etc.) and bring it with you to your appointment.

How do I pay?

Two separate payments are required (fees below are per application):

  1. Check or money order payable to the U.S. Department of State This fee must be a personal check, money order, or cashier’s check. Starter checks are not accepted (name and account number must be imprinted on the check).
    • Passport book (valid for all forms of travel): $100
    • Passport card (only valid for land and sea entry from Canada, Mexico, the Caribbean, and Bermuda): $15
    • Routine service, no additional charge. Current processing times are available on the US Department of State's website
    • Expedited service (For the most current processing times, please check the US Department of State's website): $60 (this is in addition to the fees above)
    • Expedited return shipping (passport in hand, 1-2 days after it is mailed): $22.05 (this is in addition to the fees above)
  2. Payment to Larimer County Clerk & Recorder
    • Execution fee payable by cash, check (made out to Larimer County Clerk), or credit card (3.5% processing fee applies to all card transactions): $35
    • Photo fee (if applicable): $15

  • Each appointment can include up to 3 applicants.
  • If your party has more than 3 applicants, please schedule back-to-back appointments.
  • Schedule your appointment
  • Please arrive on time for your appointment and bring all required items. If you are late or missing items, your appointment may need to be rescheduled
