First-time requests must be submitted with either of the following documents:
- A completed DR 2219 Persons with Disabilities Parking Privileges Application for individuals
- A completed DR 2218 Business Entity Disability Plates and Placard Application for businesses.
Renewal notices for individuals indicate if a self-certification or a new application is required. If you did not receive a renewal notice, please call us at 970-498-7878, option 5.
Business entities must submit a new application every three years.
Applications can be submitted online, by phone, by mail, or in person.
