Eligibility

Access to LINC will only be granted to approved employees from the following Larimer County (LC) departments and partner agencies:

  • LC Community Justice Alternatives
  • SummitStone Health Partners
  • LC Sheriff’s Office
  • LC Human Services

The type of data (e.g. HIPAA, Part 2, unregulated data) that each user can access within LINC will depend on their role, associated permissions, and consent of the individual.

How to Request Access

To request access to LINC, please follow these steps.

Step 1: Complete the LINC Access Request Form

You, as the future LINC user, will need to complete a LINC Access Request Form online.

Your submitted form will be sent to the LINC Administrator, who will verify your submitted information with your supervisor and the LINC Governance Board Member for your agency. This verification step may take up to one week to complete.

Note: Requesting users must submit their own forms. Forms submitted on behalf of another person will not be processed.

Step 2: Await Approval Notification

The LINC Administrator will notify you by email if your request has been approved or denied.

  • If your request is denied, the reason for denial will be provided.
  • If your request is approved, you can continue to Steps 3 below.

Step 3: Follow Instructions for First-Time User

If your access is approved, you will receive an email detailing how to log into the system for the first time. You will need to watch two brief training videos prior to accessing the system.
