To receive the wellness rate in 2025, it is required that the employee and/or spouse on the plan has met the following criteria:
- Make a biometric screening appointment with the Wellness clinic, or an annual physical with your primary care physician. If enrolled in the medical plan, spouses can participate too.
- Bring a screening form with you for the provider to complete and sign.
- Appointment needs to be completed between 10/1/2024 and 9/30/2025.
- Upload the completed form in Navigate.
- Meet at least 3 of the 5 Biometric Results, or Complete an Alternative Course in Navigate.
- Waist circumference of ≤35 for women & ≤40 for men, OR complete an In-Body Scan at the Wellness Clinic
- Blood Pressure below 130/80 mmHg
- Total Cholesterol below 200 mg/dL
- Triglycerides below 150 mg/dL
- Blood Glucose below 100 mg/dL or A1C at or below 7.0
- Self-report that you’re a non-tobacco user in Navigate or complete Tobacco Cessation Program on the Navigate portal.*
- Complete the wellbeing survey in Navigate.*
- Complete 2 preventative care screenings and enter in Navigate.*
