If the requestor fails to attach the appropriate court order authorizing release, the request will automatically be denied.

- The regular business hours for release of records is Monday through Friday 8:00 a.m. to 4:00 p.m.
- Larimer County considers the confidentiality of its records a priority under federal and state laws but recognizes the right of access to the records.
- All requests for copies of Larimer County records must be submitted on the Adult Protection Records Request Form filled out completely, and signed. The form can be:
- Mailed to Records Requests, 1501 Blue Spruce Drive, Fort Collins, CO 80524
- Dropped off at 1501 Blue Spruce Drive;
- Or emailed to RecordsRequests@larimer.org.
- NOTE: If you mail or email the Records Request form, you MUST include a legible copy of your Driver's License or Photo ID.
- To better serve you, please provide a description of the requested records. Be as detailed as possible.
- Your request will be processed in the order it was received.
- Once your request has been processed, you will receive a call or an email indicating when the records are ready to be picked up and the cost for obtaining the records. If you live out of state, you can mail in your money order for the amount owed and we will mail the documents to you.
- The charge for record retrieval is $40 per hour but there is no charge for the first hour of work. A $50 deposit may be required for record requests with an estimated cost over $100. All records are sent electronically. If a physical copy is required, there will be a charge of $0.25 per page, in addition to the standard hourly fee for record retrieval. Payment for records must be made in full before they are released.
- Fees can be paid online with a card through the Human Services MSB Online Payment Portal, in exact cash, or by money order made payable to Larimer County DHS.
