Omada is available to all employees enrolled in a Larimer County medical plan (both Surest or the HDHP). If you’re clinically eligible, you’ll participate in the program at no cost to you.
With Omada, you’ll receive one-on-one health coaching tailored to your goals, a connected smart scale shipped directly to your home at no cost, and easy food tracking using photos, barcode scanning, voice entry, or manual logging. The scale is yours to keep, requires no setup, and automatically syncs your progress. You’ll also have access to OmadaSpark AI for personalized answers to nutrition questions and ideas for healthier meals and snacks, along with a personalized plan that adapts as you make progress.

What is Omada?
Omada® is a virtual health program that supports lifestyle changes to help members manage weight and chronic conditions. Eligible members using a GLP-1 for weight loss also qualify for GLP-1 companion program support.
How does Omada support my weight loss journey?
Research shows that GLP-1 medication works best when paired with a lifestyle change program like Omada. In fact, members who participated in the Omada GLP-1 companion program achieved, on average, 28% greater weight loss at four months compared to those on a GLP-1 for weight loss receiving Omada's standard care.
How much does it cost?
Omada, including the GLP-1 companion program, is available at no cost to you if you're enrolled in one of the Larimer County medical plans and you are clinically eligible to enroll.
Can family members join this program?
Adult family members who are covered under a qualifying health plan and meet the clinical enrollment criteria may be eligible. Refer to your health benefit documents for details.

Why do I need Omada if I already see a doctor for my GLP-1 medication?
Omada offers care between doctor visits. Your health coach and clinical specialist are available to answer any questions you may have about GLP-ls, including managing side effects. If you meet the necessary criteria, you'll also have access to a dedicated physical activity specialist. Additionally, you can join online GLP-1 peer communities to ask questions and learn from others' weight loss experiences.
Will my medication information be safe?
Omada takes your personal health information seriously. Your participation in the program is confidential and we follow all federal and state privacy regulations as a health care provider. If I previously enrolled in Omada, do I need to re-enroll to access GLP-1 support?
If you're already enrolled in Omada for Prevention & Weight Health® or Hypertension, there's no need to enroll again. Simply log in and complete the requirements. If you've completed the program in the past, you'll need to reapply.
