Massage & Acupuncture Reimbursement Claims

Effective June 1, 2026, out-of-network massage services will no longer be covered as a benefit. Services received on or before May 31, 2026 remain eligible for reimbursement. For additional details about this change, please refer to the Inside Information article.

If you are submitting a claim for reimbursement, please follow the instructions and use the required claim form provided on this page. A completed pdf claim form must be included with your submission, along with your receipt(s) of service. Claims submitted without the required pdf form will be declined by Surest, so it’s important to ensure all documentation is included at the time of submission.

Once the form is completed and all required documentation is gathered, submit your claim through the Surest website or app.

If you would like additional support, you may complete the voluntary supplemental form below. This form allows the Benefits Team to help track your claim, assist with follow-up, and help resolve any issues that may arise during processing. Please note that completion of this form is not required to submit a claim.
